Job Summary
RN Case Manager delivers skilled nursing care and case management in patients' homes and community settings. Responsibilities include developing, coordinating, and documenting individualized care plans through a multidisciplinary team, maintaining timely documentation, and serving as a patient advocate. The role mentors newly hired nurses, ensures compliance and quality of care, completes assessments (including OASIS) per regulations, and uses EPIC charting. It requires scheduling management, collaboration with providers and support services, and contributing to continuous quality improvement; offers sign-on and relocation benefits and a compensation range aligned to hours and experience.
Required Qualifications
- Associates degree in Nursing required
- New Mexico RN License required
- BLS Required at time of Hire
- Must have current valid driver's license and auto insurance
- Minimum 1 year in direct patient care setting or home healthcare
