Job Summary
Utilize the nursing process to deliver one-on-one patient care, including assessment, diagnosis, outcomes planning, implementation, and evaluation for clients in the home setting. Document human responses to actual or potential health problems and coordinate the continuum of care by collaborating with patients, families, and inter-professional colleagues such as physicians. Implement interventions across all phases of preventative care, health maintenance, diagnosis, treatment, and follow-up while adhering to state Nurse Practice Act standards. Maintain required certifications including OASIS Specialist - Clinical (COS-C) and ensure competency in age and population-specific nursing standards.
Required Qualifications
- Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state
- Valid driver's license required
- Basic Life Support (BLS) required
- An accepted offer will require a drug screen and pre-employment background screening as a condition of employment
Desired Qualifications
- Graduate from a nationally accredited nursing program preferred, including, but not limited to, Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA)
- Bachelor's degree in nursing preferred
- Minimum of one year of professional nursing experience preferred
- Certificate for Outcome and Assessment Information Set (OASIS) Specialist - Clinical (COS-C) preferred
