RN Home Health Weekend

Perform skilled nursing visits and coordinate client care as the primary point of contact for all disciplines involved with patient treatment. Oversee visit frequency for each episode, consult with physicians and the office regarding patient care details, and ensure accurate OASIS collection with current medication profiles. Report lab values to the physician timely and attend weekly case conferences and monthly case manager meetings. This full-time weekend role requires Saturday and Sunday availability; candidates must be currently licensed RNs with at least one year of clinical experience, a valid state driver's license, and automobile liability insurance.
