Job Summary
Perform skilled nursing visits, coordinate client care, and oversee visit frequency for home health episodes. Serve as the point of contact for all disciplines involved in patient care, consulting with physicians and the office on care details. Complete accurate OASIS collections, ensure current medication profiles, and report lab values to the physician timely. Attend weekly case conferences and monthly case manager meetings. Requires current RN licensure, one year of clinical experience, a valid driver's license, and automobile liability insurance. Full-time weekend schedule with Saturday and Sunday required.
Required Qualifications
- Be currently licensed as a Registered Nurse (RN) in the state of employment
- Possess a valid state driver's license and automobile liability insurance
- Saturday and Sunday are required
Desired Qualifications
- A minimum of one year of clinical experience is preferred
