Job Summary
Full-time weekend RN in home health/hospice delivering skilled nursing visits and coordinating patient care. Serves as the point of contact for all disciplines involved with patient care, oversees visit frequency for episodes, consults with physicians and office staff about patient care details, and performs accurate OASIS data collection with current medication profiles. Ensures timely reporting of lab values to physicians and attends weekly case conferences and monthly case manager meetings. Requires current RN license in the state of employment, and a valid driver’s license with automobile liability insurance; minimum one year of clinical experience is preferred. Weekend availability with Saturday and Sunday required.
Required Qualifications
- Be currently licensed as a Registered Nurse (RN) in the state of employment
- A minimum of one year of clinical experience is preferred
- Demonstrate knowledge and skill in current nursing practice
- Possess a valid state driver’s license and automobile liability insurance
