Job Summary
Develop individualized plans of care based on physician orders and initial assessments while collaborating with physical, speech, and occupational therapists. Provide nursing care according to treatment plans, observe and report client condition changes, and offer ongoing instruction to families or other health team members. Monitor and document patient conditions and services via electronic charting, utilizing Roger's AI support to complete charting during visits. Requires a current Missouri or Compact RN license, with Home Health or Critical Care experience preferred. Driver's license and vehicle with insurance are mandatory. Full-time, part-time, and PRN availability exist in Kansas City, MO. Phoenix offers multiple major medical plans, paid mileage, and a supportive culture focused on work-life balance.
