Job Summary
Deliver high-quality, personalized nursing care in the patient's home while developing and dynamically adjusting individualized care plans. Collaborate with a multidisciplinary team of physicians, therapists, and support staff to ensure holistic care, and serve as a vital resource for patient and family education. Oversee your caseload by leading and nurturing LPNs, LVNs, and Home Health Aides to foster a cohesive care team. Utilize leading-edge tools and best practices to optimize care delivery and maintain precise documentation. Maintain a 1:1 patient-clinician ratio with ample, undivided time for each patient from initial visit through recovery.
Required Qualifications
- Graduate of an accredited nursing program
- Active RN license in the state served
- Minimum of 1 year of nursing experience
- Exceptional critical thinking
- Proven clinical expertise
- Strong case management abilities
- Unwavering commitment to patient-centered care
- Current CPR/BLS certification
- Valid driver's license
- Reliable transportation
