Conduct routine physical and psychosocial assessments, formulate individualized care plans, and adapt educational programs for patients and families. Perform skilled procedures, make independent clinical decisions, and ensure efficiency in care delivery while adhering to safety regulations. Collaborate within an interdisciplinary team to manage transitional health care needs and advocate for patient rights. This role requires an Associate of Arts in Nursing with one to three years of acute care experience and two years in a home health setting, plus Oasis C data set proficiency. Candidates must hold a California license, Basic Life Support certification, and a valid driver's license with auto insurance. Mercy Medical Center Redding serves as a Level II trauma center and the only Joint Commission-certified Advanced Thrombectomy-Capable Stroke Center north of Sacramento.
