Registered Nurse (RN) to provide in-home health visits in the Leesburg, FL area. Responsible for planning, coordination, and assurance of quality care to patients and families; develops work methods to facilitate high-quality care; delivers nursing services in clients' homes and provides case management for primary patients; monitors client needs and clinical outcomes; communicates with patients, families, physicians, and teams; participates in interdisciplinary conferences; develops and implements a plan of care with patient input; completes clinical documentation including OASIS and care coordination; collaborates with physicians and discharge planners and educates patients/caregivers; liaises with community health resources (DME, oxygen, infusions); engages in ongoing education and professional development; flexible scheduling with local territory assignments; benefits include PTO, 401(k) match, health insurance, and CEUs.
