Job Summary
Direct nursing care for home health patients through assessment, planning, implementation, and evaluation. Collect health data, analyze problems, and develop individualized care plans with prescribed interventions. Coordinate care with interdisciplinary teams, delegate tasks to unlicensed personnel, and document all relevant data per hospital standards. Conduct discharge planning, provide illness and injury prevention education to patients and families, and maintain strict confidentiality. Evaluate patient progress toward expected outcomes and participate in continuous quality improvement and performance improvement processes. Implement infection control, safety, and risk management programs while adapting to varied age-specific populations. Complete mandatory annual learning modules, attend departmental meetings, and accept assignments as a preceptor for new staff. Ensure supplies and equipment remain available and in working order.
Required Qualifications
- Associates Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) from an accredited school
- Completion of education to obtain Licensure as a Registered Nurse (RN)
- ACLS certification
- BLS
- Position requires fluency in English language to function in job environment appropriately
- Ability to lift 10 or more pounds
- Ability to sit/stand for long periods of time while completing work on a computer
- Ability to move throughout the workplace
Desired Qualifications
- Prefer 2 years of nursing experience in an acute care setting
- PALS Certification
