Job Summary
RN | Home Health in Taunton, MA with flexible scheduling. Deliver skilled nursing and patient-centered care in patients' homes, perform comprehensive physical assessments, educate patients and families, and develop/adjust care plans in coordination with physicians. Coordinate support from LPNs, HHAs, and Personal Care Attendants; arrange referrals for Physical Therapy or Medical Social Work; submit detailed clinical documentation; ensure HIPAA compliance. Preferred experience includes OASIS and WellSky (Kinnser) and expertise in medical-surgical and wound care. Requires MA RN license, TB test within 12 months, current CPR certification, and a valid driver’s license; orientation at the Taunton office is required.
