Assess patient preferences and barriers to involvement, then develop and document individual plans of care with defined goals in collaboration with the interprofessional team and patient or family. Evaluate changes in patient condition, inform caregivers, and update the plan of care in the EMR while ensuring evidence-based practice and quality outcomes. Promote respect and empathy in interactions with diverse populations, integrating data from the team to provide safe, timely care. This role includes a 40-hour flexible week with 10-hour shifts, paid on-call nights, and a sign-on bonus, serving thousands of patients across Missouri and Illinois through BJC Home Care's largest home care network.
