Job Summary
Assess and coordinate patient discharge planning needs while traveling to patient homes, community-based residential facilities, assisted living locations, hospitals, or other sites. Perform skilled nursing care including vitals, wound care, IV care, ostomy care, blood draws, injections, and physical assessments. Collaborate with physicians and multidisciplinary teams to develop comprehensive care plans, update status, and document visits in adherence to regulations. Complete admission paperwork, verify insurance, and perform point-of-care testing as assigned. Respond to calls from patients and families, complete follow-ups, and ensure required visits are completed timely.
Required Qualifications
- Registered Nurse (RN)
- Basic Life Support HealthCare Provider (BLS HCP) - American Heart Association (AHA)
- Proof of motor vehicle liability insurance that meets minimum state requirements
- Driving Record - Motor Vehicle Report (MVR)
- Driver License – State Issued
- Graduate of accredited school of nursing or education equivalency for licensing
