Job Summary
Administers skilled nursing care in patients' homes; conducts initial evaluation visits and regularly reevaluates nursing needs; initiates and revises the plan of care; performs preventive and rehabilitative nursing procedures; prepares clinical and progress notes; counsels patients and families; processes orders and notifies physicians of changes; coordinates referrals to Physical Therapy, Speech-Language Pathology, Occupational Therapy, and Medical Social Work; supervises and teaches other nursing personnel; conducts patient care conferences; participates in on-call duty nights, weekends, and holidays; completes and submits OASIS assessments and discharge summaries; uses ICD-10 codes; travels within Pierce County; requires WA RN license.
Required Qualifications
- Current Registered Nursing license in Washington state.
