RN - Home Health - Part Time - Washington

Provide day-to-day patient care in the home setting, including case management, admissions, post-hospitalization assessments, recertifications, discharges, patient teaching, dressing changes, IV therapy administration, and access site care. Ensure safety and appropriate care by assisting with lifeline services, meals on wheels, and public program instruction. Document all duties timely using the Epic computer system. Perform visits on an as-needed basis following department policies. Requires a current Missouri RN license, BLS certification, and valid driver's license. Full-time day shift opportunities include rotating on-call weekends and holiday rotations.
