RN - Home Health - Part Time - Washington

Conduct day-to-day patient care in the home setting, including case management, admissions, post-hospitalization assessments, recertifications, discharges, patient teaching, dressing changes, IV therapy administration, and access site care. Provide appropriate documentation via the Epic computer system and assist with safety needs such as lifeline services, meals on wheels, and public program instruction. Perform duties on an as-needed basis following department policies. Requires current Missouri RN license, BLS certification, and valid driver's license. Full-time day shift (8:00 AM–4:30 PM) with rotating on-call weekends; part-time per diem available.
