Job Summary
Deliver skilled nursing care in the home health setting as directed by a physician. Assess patient needs, develop individualized care plans, monitor progress, document services, and educate patients and families on condition management, medications, and recovery. Collaborate with physicians and interdisciplinary teams to ensure continuity of care. Full-/part-time day shifts in the St. Louis Metro area and neighboring counties with autonomy and meaningful patient-centered care in a community setting. Includes mileage reimbursement and rotating on-call weekends/holidays. Requires Missouri RN licensure, BLS, and a valid driver’s license.
Required Qualifications
- Graduation from an accredited nursing program
- Current RN license in Missouri
- BLS Certification through the American Heart Association
- Valid driver’s license and proof of auto insurance
