RN - Home Health - Part Time

Conduct home health visits and coordinate client care while serving as the primary point of contact for all disciplines involved in patient treatment. Perform accurate OASIS collection, maintain current medication profiles, and ensure timely reporting of lab values to the physician. Consult with the office regarding patient care details, attend weekly case conferences, and participate in monthly case manager meetings. This part-time salaried role requires 24 hours of work over four days per week and includes eligibility for a 60% paid days off program.
