Perform Start of Care, Resumption of Care, Recertification, Transfer, and Discharge OASIS assessments while conducting comprehensive nursing assessments and developing individualized plans of care. Coordinate and manage patient care throughout the episode, providing skilled nursing services, patient education, medication management, and disease management. Collaborate with physicians, therapists, and social workers to monitor progress and adjust care plans as needed. Complete accurate OASIS documentation and clinical records in accordance with Medicare and agency guidelines. Participate in quality improvement initiatives and maintain regulatory compliance.
