RN - Home Health - Mercer County

Perform Start of Care, Resumptions of Care, Recertification, Transfer, and Discharge OASIS assessments while conducting comprehensive nursing assessments and developing individualized plans of care. Coordinate and manage patient care throughout the episode of care, providing skilled nursing services, patient education, medication management, and disease management. Collaborate with physicians, therapists, social workers, and other interdisciplinary team members to monitor progress and adjust care plans as needed. Complete accurate and timely OASIS documentation and clinical records in accordance with Medicare, state, and agency guidelines. Participate in quality improvements initiatives and maintain compliance with regulatory standards. This role offers weekly pay, pay per visit, and no on-call rotation.
