Job Summary
Provide in-home direct patient care per prescribed plan of care and nursing scope of practice; monitor patient progress; administer medications, insulin, and IV fluids; inspect and care for wounds and assist with personal hygiene; assess for complications and deliver appropriate treatments; collaborate with providers and team to ensure quality outcomes; positions involve 1:1 patient care in home health settings across WV service locations (Bridgeport, Kingwood, Grafton, Philippi).
Required Qualifications
- Education: Associate degree in Nursing (AND) or Associate of Applied Science in Nursing (AAS) required
- Licensure: Current unrestricted RN license in the state of assignment
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to/from care sites
- Practical trach and/or ventilator experience preferred, not required
