Job Summary
Conduct home health visits and coordinate client care as the primary point of contact for all disciplines involved in patient treatment. Oversee visit frequency for each episode, consult with physicians regarding care details, and perform accurate OASIS data collection. Maintain current medication profiles, ensure timely reporting of lab values to the physician, and attend weekly case conferences and monthly case manager meetings. Requires current RN licensure, minimum one year clinical experience, and valid driver's license.
