Job Summary
Conduct home health visits and coordinate client care while serving as the point of contact for all disciplines involved in patient treatment. Oversee visit frequency for each episode, consult with physicians and the office regarding care details, and perform accurate OASIS collection. Maintain current medication profiles, report lab values to the physician timely, and attend weekly case conferences and monthly case manager meetings. Requires current RN licensure, minimum one year clinical experience, valid driver's license, and automobile liability insurance. Full-time role with $5,000 sign-on bonus, 401(k) matching, and comprehensive medical, dental, and vision coverage.
Required Qualifications
- Be currently licensed as a Registered Nurse (RN) in the state of employment
- Possess a valid state driver's license and automobile liability insurance
Desired Qualifications
- A minimum of one year of clinical experience is preferred
