Job Summary
Conduct home health visits and coordinate client care while serving as the primary point of contact for all disciplines involved in patient treatment. Manage visit frequency per episode, consult with physicians and the office regarding care details, and perform accurate OASIS collection. Maintain current medication profiles, ensure timely lab value reporting, and attend weekly case conferences and monthly case manager meetings. Requires current RN licensure, minimum one year clinical experience, and valid driver's license. Required Qualifications
- Be currently licensed as a Registered Nurse (RN) in the state of employment
- Possess a valid state driver's license
