Job Summary
Conduct home health visits and coordinate client care, serving as the primary point of contact for all disciplines involved in patient treatment. Oversee visit frequency per episode, consult with physicians regarding care details, and perform accurate OASIS collections. Maintain current medication profiles, ensure timely lab value reporting to the physician, and attend weekly case conferences and monthly case manager meetings. Requires current RN licensure, minimum one year of clinical experience, and a valid driver's license. Required Qualifications
- Be currently licensed as a Registered Nurse (RN) in the state of employment
