Job Summary
Conduct home health visits and coordinate client care while serving as the point of contact for all disciplines involved in patient treatment. Oversee visit frequency for episodes, consult with physicians and the office regarding care details, and perform accurate OASIS collection. Ensure medication profiles remain current, report lab values to the physician timely, and attend weekly case conferences and monthly case manager meetings. Maintain a valid state driver's license and automobile liability insurance.
Required Qualifications
- Be currently licensed as a Registered Nurse (RN) in the state of employment
- Possess a valid state driver's license and automobile liability insurance
Desired Qualifications
- A minimum of one year of clinical experience is preferred
