Job Summary
Deliver high-quality, personalized nursing care in a patient's home environment while maintaining a 1:1 clinician-to-patient ratio. Develop and dynamically adjust individualized care plans to promote patient independence, collaborating seamlessly with physicians, therapists, and support staff for holistic outcomes. Serve as a primary resource for patient and family education, providing emotional support and guiding the full arc of healing from initial visit to recovery. Oversee caseloads by mentoring and guiding LPNs, LVNs, and Home Health Aides to foster a cohesive care team, utilizing leading-edge tools for optimized delivery and precise documentation. This PRN role offers structured growth paths for clinical leadership, operations, or educational initiatives within a faith-based ministry dedicated to superior patient outcomes.
Required Qualifications
- Graduate of an accredited nursing program
- Active RN license in the state served
- Minimum of 1 year of nursing experience
- Exceptional critical thinking
- Proven clinical expertise
- Strong case management abilities
- Unwavering commitment to patient-centered care
- Current CPR/BLS certification
- Valid driver's license
- Reliable transportation
