RN Home Health Clinical Manager

Lead and oversee home health patient care with direct patient care and leadership oversight of designated home health personnel in the assigned territory. Responsibilities include managing nursing staff (scheduling, coverage, performance), coordinating referrals/admissions/care transitions, developing individualized care plans, providing direct patient care (assessments, symptom management, education), communicating with physicians, ensuring documentation and regulatory compliance (OASIS, care plans, orders), managing EMR workflows, monitoring caseloads and staffing, leading team meetings and care conferences, coaching and evaluating staff, supporting QAPI and process improvement, assisting with budgeting and reporting, and building community partnerships for referrals and growth. Requires MN RN licensure, 5 years nursing experience, home health care experience; OASIS certification preferred; management experience preferred. Valid MN driver’s license and access to reliable vehicle.
