Perform Start of Care, Resumption of Care, Recertification, Transfer, and Discharge OASIS assessments while conducting comprehensive nursing assessments to develop individualized plans of care. Coordinate and manage patient care throughout the episode, providing skilled nursing services, patient education, medication management, and disease management. Collaborate with physicians, therapists, social workers, and other interdisciplinary team members to monitor progress and adjust care plans as needed. Complete accurate and timely OASIS documentation and clinical records in accordance with Medicare, state, and agency guidelines. Participate in quality improvement initiatives while maintaining compliance with regulatory standards. Deliver exceptional care where patients feel most comfortable with manageable caseloads and no on-call rotation.
