(RN) Home Health Case Manager- (Citronelle/Eight Mile, AL) - ProHealth Home Health

Administer skilled nursing care to patients on an intermittent basis in their place of residence in accordance with physician orders and a plan of care. Make initial evaluation visits, regularly reevaluate patient needs, and initiate or revise the plan of care as required. Provide services requiring specialized nursing skills, including preventive and rehabilitative procedures, while preparing clinical notes and care conference summaries. Coordinate services, inform personnel of condition changes, and counsel patients and families on meeting nursing needs. Process orders, complete certification and discharge summaries, and determine the amount and type of nursing needed for each patient. Refer patients to physical, speech, and occupational therapists as necessary. Participate in inservice programs, peer reviews, and quality assessment initiatives. Take on-call duty nights, weekends, and holidays as assigned.
