Job Summary
RN in-home health case manager provides skilled nursing care to patients in their residences under the plan of care, with initial evaluations and ongoing reevaluations, initiates and revises the plan of care, delivers services requiring specialized nursing skills, documents care as clinical/progress notes, coordinates services, informs staff of patient changes, counsels patients and families, participates in inservice programs, processes orders and communicates with physicians, determines nursing needs per patient, refers to PT/SLP/OT/MSW when needed, supervises other nursing personnel, conducts patient care conferences, participates in peer review and PI activities, and may take on-call duties; requires travel to patient homes, adheres to agency policies, and uses ICD-10 codes; environment includes work in patient homes and agency office.
Required Qualifications
- Graduate of an approved school of professional nursing
- Current RN license in the state(s) in which practicing
- Two (2) years nursing experience preferred
- Acceptance of philosophy and goals of this Agency
- Ability to exercise initiative and independent judgment
- Ability to supervise and teach other nursing personnel
- Ability to provide total patient care as needed
- Completion of OASIS assessments may be inferred from job responsibilities
Desired Qualifications
- Two (2) years’ nursing experience, preferred
- Graduate of an approved school of professional nursing
- Currently licensed in the state(s) in which practicing
- Acceptance of philosophy and goals of this Agency
- Ability to exercise initiative and independent judgment
