Job Summary
Conduct initial patient and family assessments to determine home care needs, including complete physical exams and history of current and previous illnesses. Initiate and revise plans of care based on nursing diagnoses, developing goals that incorporate therapeutic, preventive, and rehabilitative actions while involving patients and families. Administer medications and treatments as prescribed, counsel patients on meeting nursing needs, and provide health care instructions. Coordinate discharge planning and act as Case Manager to oversee assigned caseloads when directed. Prepare clinical notes, communicate with physicians regarding patient changes, and supervise home health aides every two weeks. Participate in on-call duties and ensure availability of equipment and services. Requires current RN licensure, National League for Nursing accredited education, CPR certification, and a licensed insured vehicle. Minimum two years nursing experience preferred, with at least one year in public health or home care. Must be willing to travel beyond 25 miles from residence occasionally for start of care.
Required Qualifications
- Registered nurse with current licensure to practice professional nursing in the state
- Graduate of National League for Nursing accredited school of nursing
- Current CPR certification
- Licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
- Self directed and able to work with minimal supervision

Desired Qualifications
- Minimum of two years experience, at least one of which is in the area of public health or home care nursing
