Job Summary
Conduct initial and regular patient assessments, develop comprehensive care plans, and administer prescribed medications and treatments. Initiate preventive and rehabilitative nursing procedures while counseling patients and families on health instructions and discharge planning. Coordinate care by communicating with physicians and community health resources, and act as Case Manager to oversee assigned caseloads. Supervise and evaluate home health aide care every two weeks, ensure equipment availability, and participate in on-call duties. Requires current RN licensure, CPR certification, and willingness to travel beyond 25 miles for start-of-care.
Required Qualifications
- Registered nurse with current licensure to practice professional nursing in the state
- Graduate of National League for Nursing accredited school of nursing
- Current CPR certification
- Licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
- Self directed and able to work with minimal supervision
- Excellent observation skills

- Nursing skills per competency checklist
- Ability to organize and prioritize workload independently
- Ability to lift, position, or transfer patients
- Ability to lift supplies and equipment
- Visual acuity and hearing to perform required nursing skills
- Willingness to travel beyond 25 miles from residence on occasion to complete start of care
Desired Qualifications
- Minimum of two years experience, at least one of which is in the area of public health or home care nursing
