RN - Home Health Care

Conduct initial and ongoing patient assessments to determine home care needs and nursing diagnoses. Develop comprehensive care plans incorporating therapeutic, preventive, and rehabilitative actions while including patients and families in the planning process. Initiate appropriate nursing procedures, administer prescribed medications, and provide health care instructions. Act as Case Manager when assigned to coordinate patient care for the caseload. Prepare clinical notes, update physicians, and communicate with community health personnel to coordinate care. Participate in on-call duties and instruct, supervise, and evaluate home health aide care every two weeks.
