RN - Home Health

Registered Nurse (RN) to provide in-home patient care in Worcester, MA. Responsibilities include comprehensive patient assessments, developing and updating individualized care plans with physicians and the interdisciplinary team, skilled nursing services (medication management, wound care, disease management, patient education), accurate documentation of care provided, coordinating with therapists and other healthcare professionals, educating patients and caregivers on disease processes and self-care, ensuring compliance with Medicare Conditions of Participation and state regulations, and participating in quality improvement activities and case conferences. Requires active RN license in the state of practice, a minimum of 1 year of clinical nursing experience (home health preferred), strong assessment and communication skills, ability to work independently, valid driver's license with reliable transportation and current auto insurance.
