Assess, plan, implement, document, and evaluate outcomes for patients in their homes under physician direction. Manage caseloads of individuals with injuries, illnesses, or chronic conditions by providing pertinent education, treatment, and emotional support. Demonstrate technical proficiency in anticipating and responding to changes in clinical status while maintaining excellent rapport with patients and family members. Communicate consistently with doctors and healthcare professionals regarding medication and plan of care changes. Requires current Missouri RN license, BLS CPR certification, and ability to drive.
