Assess, plan, implement, document, and evaluate outcomes for skilled nursing services provided to patients in their homes under physician direction. Manage a caseload of patients with diverse health conditions, delivering pertinent education, treatment, and emotional support to aid recovery or chronic disease management. Demonstrate assessment skills and technical proficiency while anticipating changes in clinical status, maintaining excellent rapport with patients and families, and communicating consistently with doctors and healthcare professionals regarding medication and care plans. Requires current Missouri RN license, BLS CPR certification, and ability to drive.
