Assess, plan, implement, document, and evaluate outcomes for skilled nursing services provided to patients in their homes under physician direction. Manage a caseload of patients with diverse health conditions, delivering pertinent education, treatment, and emotional support to aid recovery or manage chronic disease. Demonstrate technical proficiency in anticipating clinical status changes while maintaining excellent rapport with patients and family members. Communicate consistently with doctors and healthcare professionals regarding patient care, ensuring accurate documentation of medication and plan of care adjustments. Requires current Missouri RN license, BLS CPR certification, and ability to drive.
