Assess, plan, implement, document, and evaluate outcomes for patients in their homes under physician direction. Manage a caseload of individuals with chronic conditions, injuries, or illnesses by providing pertinent patient education, treatment, and emotional support. Demonstrate excellent assessment skills and anticipate changes in clinical status while communicating with doctors and family members regarding medication and care plans. Maintain accurate documentation of all patient interactions and care changes. Requires current Missouri RN license, BLS CPR certification, and a valid driver's license.
