RN-Home Care

Provide skilled care and case management to patients and families in accordance with the State Nurse Practice Act and professional standards. Utilize nursing theory to communicate observations, document assessments, and promote health through direct care, teaching, and counseling. Maintain patient well-being via appropriate rehabilitative measures while reporting to the VNA Clinical Team Manager within established organizational policies. Operate onsite at 168 Industrial Drive with a 40-hour weekly day shift, requiring a current Massachusetts RN license, BSN, and one year of medical/surgical experience.
