Job Summary
Assess patients with complex medical conditions and develop tailored care plans for home health or hospice services. Educate patients and families on care strategies, resources, and expectations for home-based recovery. Coordinate timely referrals to VNA home care services, ensuring smooth transitions and continuity of care. Collaborate with clinicians and external providers to optimize patient outcomes while documenting condition changes in real time. Participate in a comprehensive two-week clinical orientation covering EMR systems, safety protocols, and compassionate care practices. Requires current RN license in New Jersey, minimum two years of experience in home health nursing or clinical care coordination, and ability to prioritize in a fast-paced hospital setting.
Required Qualifications
- Current RN license in New Jersey
- Graduation from an accredited nursing program
- Minimum of 2 years of experience in home health nursing or clinical care coordination
- Strong communication skills
- Ability to prioritize and stay organized in a fast-paced hospital setting
