RN-Home Care- Full Time Weekends *Sign On*

Job Summary
Provides day-to-day patient care in the home setting including case management and total care of multiple patients with different levels of acuity, admissions to home care services, post-hospitalization assessments, recertifications, discharges, patient teaching, IV therapy administration, access site care and dressing changes; uses Epic for documentation; per diem visits on an as-needed basis following policies; requires lifting up to 50 lbs, ability to transfer patients weighing up to 100 lbs, and driving up to 50 miles; must meet CMS Rules and Regulations for Home Care; weekend/full-time RN home care role with a sign-on incentive.
Required Qualifications
- Graduation from accredited school of nursing
- RN license in applicable state of practice
- Valid driver's license in applicable state of practice
- Automobile liability insurance as required by state
- Three years nursing experience with minimum one year in hospital setting; preferably home care
- CPR biannually
