RN Home Care Case Manager

Job Summary
Conduct comprehensive new patient assessments and scheduled re-assessments in clients' homes throughout the Chicago area. Manage an active caseload by coordinating care plans, updating documentation, and ensuring continuity of care while serving as a clinical point of contact for field nurses. Communicate with physicians, families, and interdisciplinary team members regarding patient status and escalate clinical concerns appropriately. Ensure all care plans and documentation meet regulatory and Joint Commission compliance standards. Participate in office-based team meetings and case conferences approximately one day per week. Requires an active Illinois RN license, critical care experience, and a reliable vehicle for extensive daily travel.
Required Qualifications
- Active, unrestricted RN license in the state of Illinois
- Critical care clinical experience
- Reliable personal vehicle
- Valid driver's license
- Current auto insurance
- Comfortable with extensive daily driving across the greater Chicago area
- Negative TB Test
- Physical
- Current CPR Certification
- Strong assessment, critical thinking, and clinical decision-making skills
- Excellent communication skills
- Proficiency with electronic documentation systems
Desired Qualifications
- Minimum 1 year of ICU experience
- Home care, hospice, or case management experience
