Job Summary

Conduct comprehensive nursing assessments, reassessments, and discharge planning for hematology/oncology and bone marrow transplant patients. Establish and evaluate care plans incorporating evidence-based practices, disease-specific protocols, and physician orders while administering medications and monitoring IV fluids. Provide specialized care for high-risk patients, including pain management, restraint alternatives, and abuse assessments, ensuring strict adherence to infection control and safety standards. Collaborate with the health team to coordinate daily care, perform accurate handoffs, and document all interventions and changes in patient status.

  • Incorporation of disease specific evidenced based practice into nursing care plan
  • Provision and documentation of nursing interventions based on assessed patient needs
  • Collaboration with appropriate health team members for coordination of daily plan of care
  • Provision, coordination and communication of patient care including accurate Handoff Communication Reports
  • Administration and documentation of medications accurately according to policies and procedures
  • Monitoring, maintenance and documentation of accurate IV fluids and parenteral nutrition
  • Completion of referrals as indicated by assessment data
  • Requesting consultation for special needs, equipment, or information
  • Provision of patient/family education and discharge planning per documentation guidelines and protocol
  • Clarification of all physician orders as warranted
  • Acknowledgment and implementation of physician orders in an accurate and timely manner
  • Assistance with physician procedures/treatments or delegation to Care Partner
  • Documentation of 'Readback' for all telephone / verbal orders
  • Taking telephone / verbal orders only in emergency situations
  • Recognition of changes in patient's condition and taking appropriate nursing actions
  • Involvement of family/guardian when providing care and in decision-making
  • Recognition of risks for patient and taking appropriate action
  • Incorporation of Infection Control practices in daily care
  • Ongoing evaluation of effectiveness of care based on assessment data
  • Evaluation and documentation of effectiveness of patient/family education
  • Evaluation of plan of care and modification as indicated
  • Recognition of significant changes in patient's clinical parameters and reporting immediately
  • Identification of problems, gathering pertinent data, suggesting solutions, communicating using appropriate lines of authority, and working toward problem resolution
  • Reporting of variation from care/treatment following the occurrence reporting policy and procedures
  • Provision of specialized care to patients at high risk for injury
  • Initiation/evaluation of alternatives to restraint prior to application
  • Application of restraints consistent with the approved procedure
  • Monitoring and assessment of patient's response throughout the restraint period at appropriate intervals
  • Provision of specified patient care (toileting, skin care, hydration, feeding, etc.) on a timely basis
  • Provision of consultation for peers to determine alternatives to restraints and 1:1 observation
  • Documentation of restraint use and associated care thoroughly
  • Assessment of patient for presence of pain on admission and during assessments/reassessments
  • Incorporation of patient's cultural/spiritual beliefs regarding pain into pain management plan
  • Implementation of pain management techniques with focus on prevention rather than treatment
  • Inclusion of patient and/or family members in developing a pain management plan
  • Consideration of other methods of pain control when developing plan of care
  • Awareness of abuse recognition criteria and incorporation into assessments
  • Reporting of signs of possible abuse/neglect to the Administrative Director of Behavioral Health, physician, Risk Management and Social Work
  • Taking appropriate action to support patient safety when signs of abuse are noted
  • Adherence to all quality and performance standards, policies, procedures, protocols when implementing clinical and technical aspects of care
  • Participation in learning experiences that increase professional competence
  • Demonstration of appropriate technical and cognitive skills for area of practice
  • Maintenance of currency in all hospital/unit information, communication, policies and procedures
  • Attendance of staff meetings/reviews of minutes when absent
