Job Summary
Provide and manage patient care in the Heart Failure Unit as an interdisciplinary team member. Assess patient status through interview, observation, and physical assessment; develop an individualized care plan with the patient/family and healthcare team; implement identified interventions to achieve outcomes; document and evaluate progress; modify plans as conditions change; ensure proper documentation and safety; perform bedside lab testing and quality control; maintain current knowledge of evidence-based nursing practices; educate patients and families; share clinical knowledge to support team members; promote safety through proper use of lift equipment and safe patient handling; collaborate on task organization from admission to discharge; contribute to process improvements and quality initiatives.
Required Qualifications
- Licensed Registered Nurse (RN) in Illinois
- Basic Life Support (BLS) certification (American Heart Association) within 30 days
- College Diploma in Nursing
