RN Heart Clinic Triage

Document patient information in the electronic health record (EPIC) following standard work guidelines while assessing and triaging patient care needs. Suggest solutions to patient care problems and complaints, and identify recommendations for departmental issues. Coordinate care for Heart Failure and general cardiology patients, including telephone triage, advising, consultation, and in-office cardiovascular procedures. Assess patient and family learning needs to coordinate teaching plans and manage care coordination with cardiologists and vendor representatives. Administer prescribed medications and treatments in accordance with nursing standards and clinic policy. This role supports the provider's practice by assisting in the coordination of patient care and promoting a constructive approach to problem solving.
