RN Heart Clinic Triage

Document patient information in the electronic health record (EPIC) and assess patient care needs by triaging individuals for Heart Failure and general cardiology. Suggest solutions to patient care problems, coordinate teaching plans for patient/family learning needs, and manage care coordination with cardiometabolic patients and vendor representatives. Administer prescribed medications and treatments in accordance with nursing standards and clinic policy while advising on in-office cardiovascular procedures. Identify and recommend solutions to departmental problems, assess departmental function and productivity, and coordinate care across the continuum for specialty services.
