RN Heart Clinic Triage

Document patient information in the electronic health record (EPIC) and assess and triage patient care needs for Heart Failure and general cardiology patients. Suggest solutions to patient care problems and coordinate teaching plans based on patient/family learning needs. Manage care coordination with cardiome patients and vendor representatives while advising on telephone triage and in-office cardiovascular procedures. Identify and recommend solutions to departmental problems, assisting in assessing departmental function and productivity to recommend necessary changes. Administer prescribed medications and treatments in accordance with nursing standards and clinic policy.
