RN Heart Clinic Triage

Document patient information in the electronic health record (EPIC) and assess and triage patient care needs for Heart Failure and general cardiology patients. Suggest solutions to patient care problems and coordinate care across the continuum for specialty services internally and externally. Teach patients and families, manage care coordination with cardiometabolic patients and vendor representatives, and administer prescribed medications and treatments in accordance with clinic policy. Identify and recommend solutions to departmental problems while assessing departmental function and productivity.
