RN Heart Clinic Triage

Document patient information in the electronic health record (EPIC) following standard work guidelines and assess and triage patient care needs. Suggest solutions to patient care problems and complaints, while identifying and recommending solutions to departmental problems. Assist in assessing departmental function and productivity, recommending changes as necessary. Assess patient and family learning needs, coordinating and carrying out teaching plans. Coordinate care for Heart Failure and general cardiology patients, including telephone triage, advising, consultation with patients, nurse-managed care coordination, patient and family education, and in-office cardiovascular procedures. Manage care coordination with cardiomeg patients and vendor representatives. Demonstrate ability to carry out skills as outlined in nursing procedures for clinical areas, including administration of prescribed medications and treatments in accordance with nursing standards and clinic policy.
